Patients can rate the services they receive at our Practice by filling in an FFT questionnaire either online or by filling in a paper form when you visit the surgery.
During COVID the need to publish these was suspended and paper forms were not recommended, however from 1st April 2022 this has now been re-launched. The online form should change soon to reflect the new question (below) but in the meantime please do feel free to download and complete the form and return to the practice.
The question patients need to answer is:
“Thinking about your recent visit overall, how was your experience of our service?”

The results of this survey will be reviewed in both our Practice Meetings and in our PLT sessions with all of our staff, alongside other feedback we receive and any comments or complaints. We will discuss feedback with our PPG and will provide update reports on this website.
